This website is published by Precision Medicine and Wellness by Elizabeth Bagan, APRN, with an office in Jacksonville, Florida. It gives general information about the practice and the services it offers.

It is not medical advice

Information on this site is educational. It does not replace a consultation, an examination, or advice about your own health, and reading it does not create a clinician and patient relationship.

Treatment is only ever offered after an evaluation. What is appropriate differs from person to person, and outcomes vary and are not guaranteed.

Suggested questions to ask your clinician are general. They do not diagnose, assess eligibility or recommend treatment.

Emergencies

This website, our email and our text line are not monitored around the clock and must not be used in an emergency. If you are having a medical emergency, call 911.

Booking, the patient portal and other services

Online booking, the patient portal, payment and financing are provided by third party companies under their own terms and privacy notices. When you use one of those services, its terms apply to what you do there.

Cancellation, deposit and rescheduling policies are confirmed at the time you book.

Payments, memberships and gift cards

Prices and availability can change. The price that applies is the one confirmed at the time of your appointment or purchase.

A membership runs for at least three months and requires a card on file. Packages carry the options and expiry dates confirmed when you buy them. Gift cards cannot be refunded and work toward any service or product.

Lab work is not billed to insurance. You pay for labs directly, and we give you the price first.

Accuracy of the information

We work to keep this site accurate and review clinical pages on a schedule. Medical knowledge, product labelling and our own services change, so information may occasionally be out of date. If you spot an error, please tell us.

Our content

The text, design and graphics on this site belong to the practice or are used with permission. You may view, print and share pages for personal, non-commercial use. Please do not copy or republish the site's content for any other purpose without asking us first.

Names of products and devices mentioned on this site belong to their manufacturers.

Using the site properly

Please do not try to disrupt the site, access parts of it you are not meant to, or use it to send unsolicited messages. Please do not send detailed medical information through a website form or by email; call the office instead.
